New radiotherapy for brain cancer better preserves cognitive function



When talking about radiotherapy to treat brain cancer, radiation therapy to the entire brain but with evasion of the hippocampus and administered in conjunction with the drug memantine better preserved cognitive function and showed oncological results similar to those of traditional radiotherapy administered to the entire brain and with memantine.



The results were presented on Tuesday, October 23, by Mayo Clinic scientists, at the annual meeting for 2018 of the American Society of Radiological Oncology in San Antonio (USA).



"The hippocampus is the part of the brain that is related to the limbic system and cognitive functions, including memory. Based on decades of translational studies, we know that even a modest dose of radiation in the brain region of the hippocampus contributes to patients having cognitive problems, such as memory loss, "says Dr. Paul Brown, radiation oncologist Mayo Clinic and expert author of the study. The doctor adds that radiation therapy to the entire brain, but that prevents hippocampus, was created to help patients with brain cancer avoid cognitive damage by receiving radiation therapy throughout the brain.



In order to conduct the study, Dr. Brown and his colleagues compared whole-brain radiation therapy with hippocampal evasion versus whole-brain radiation therapy in patients assigned to the two study arms receiving memantine.



Between July 2016 and March 2018, the scientists enrolled 518 patients, who were randomly assigned to the two branches of the study. The average age of the patients was 61.5 years. The treatment branches did not show variations in their basic characteristics.



The scientists found that patients who received whole-brain radiation but evacuated the hippocampus retained better cognitive function and both their cancer control and survival were similar to patients who received standard radiation therapy throughout the brain. They also found that although age independently predicts neurocognitive function, the neurocognitive benefit of avoiding the hippocampus did not change with age.



"The results of this trial can have a great impact on patients, since it is estimated that the incidence in the United States of metastatic brain cancer is so high that it reaches 200,000 cases per year. In addition, depending on the type of primary cancer, between 10 and 30 percent of cancer patients will develop cancer that spreads to the brain, "explains Dr. Brown.



"It is one of the few clinical trials in radiological oncology that originated in the laboratory, was tested in a phase II trial and, finally, was added to previous trials to be tested in a phase III trial, which showed positive improvements. Next month, at the 2018 annual meeting of the Society of Neuro-oncology in New Orleans we plan to present our results with updated data, which include symptoms reported by patients, "concludes Dr. Brown.
