A new international study published in the Journal of Bone Oncology, the result of collaboration between several clinical centers across Europe and in which Roger Gomis, a CIBER Cancer researcher (CIBERONC) at IRB Barcelona and ICREA has participated, has evaluated the risks and long-term benefits of adjuvant zoledronic acid (ZOL) in patients with breast cancer. The results obtained suggest that the evaluation of the status of the MAF gene in the primary tumor could provide an objective indicator to select the patients that could benefit from the treatment with adjuvant bisphosphonate, specifically zoledronic acid.



Previous studies by Dr. Gomis' group had already pointed to the MAF gene as the "director" that ignites a whole program of cellular functions that allow bone metastasis to occur in mammary tumors. In tumors that have the altered MAF transcription factor, the risk of metastasis to bone is 14 times higher than those that do not have it altered, so it is a gene that can perfectly predict metastasis to bone.

This new randomized trial, which is integrated into AZURE, one of the largest phase III studies focusing on the evaluation of adjuvant bisphosphonates, has included a total of 3,360 patients with stage II / III breast cancer, with follow-up 10 years.



The results obtained suggest that, in patients with negative MAF, the inclusion of zoledronic acid in the adjuvant environment was associated with improved results, regardless of the menopausal status. In particular, this trial highlights an improvement in 10-year survival of 31% in patients with negative MAF tumors treated with zoledronic acid.



In contrast, patients with a positive MAF tumor did not benefit from the treatment. In addition, in women with positive non-postmenopausal MAF at the start of treatment, higher adverse outcomes and mortality were observed.



"The current study could be very useful for doctors and avoid the unnecessary treatment of patients who would not benefit or could be harmed by the treatment," says Professor Gomis.
